Submitted with the application must be:
(1) Birth certificate (issued by state, county, or municipal authority);
(2) High school diploma (or equivalent);
(3) DD214 (if applicable)
(4) A current valid driver’s license
(An original transcript must be provided for credit to be given for training and/or courses above the HS level.)
